(나스닥: INO)]는 고위험 인간 유두종바이러스(HPV) 16 및/혹은 18 타입이 일으키는 전암성 질병으로서 고도 편평상피내 병변(HSIL)이라고도 하는 여성기이형성증 환자들에 대한 VGX-3100의 안전성과 효능을 평가하기 위해 설계된 오픈 레이블 2상 시험의 중간 결과를 오늘 발표했다. 전체 22명의 환자가 모두 4 도스를 주입 받았으며 본 연구 말기에 그 결과가 평가된다. 이노비오는 VGX-3100 치료 이후 6개월 시점의 일차 엔드포인트 평가를 끝낸 환자 10명에 대한 중간 데이터를 보고하는 것이다. 평가 결과에 의하면, 80%(환자 10명 중 8명)가 확정 병변 면적(평균 2㎠)을 줄였으며(평균 60% 감소) 20% 환자들(10명 중 2명)은 여성기이형성증을 완전히 해소하여 치료 후 6개월 시점에 치료 부위에서 바이러스가 검출되지 않았다.

시험 결과는 4월 1일부터 4월 2일까지 열리는 미국 질경검사 및 자궁병리학회(ASCCP) 연례 회의에서 진행될 "HPV-16 및/혹은 HPV-18(HPV-16/18)이 일으키는 여성기HSIL 치료를 위한 VGX-3100의 오픈 레이블 2상 연구 예비 결과"라는 제목의 온라인 세션에서 발표된다. 안전성 결과는 이미 알려진 VGX-3100 안전성과 일치했다. 본 의약품에 관련된 심각한 역효과는 일어나지 않았다. 암종 발생 사례는 관찰되지 않았다.

여성기HSIL는 희귀 난치성 질병으로써 보통 수술이 필요하지만 심각한 신체적 및 심리적 후유증(예컨대 엄청난 통증, 성기의 흉한 변형, 성기능 장애 등)이 있다. 수술 치료 3년 후의 고도 여성기 전암의 재발률이 약 30에서 50%에 달하기 때문에 수술 성공률은 낮다고 할 수 있다. 대체로 여성기 HSIL 환자의 5% 이하는 저절로 낫는다. 여 성기 HAIL을 적절히 치료하지 않을 경우 미국에서 2020년 한 해 동안에만 약 1천350 명이 사망할 것으로 예상되는 여성기암으로 진전될 수 있다.

여성기이형성증에 대한 VGX-3100의 안전성 및 효능 결과

여성기이형성증(HSIL)
여성기 전암은 치료하지 않을 경우 침습성 여성기암으로 진전될 수 있다. 미국에서 매년 약 2만7천 명의 HPV 관련 여성기 전암 환자가 발생하며 이와 비슷한 수의 환자가 유럽에서도 매년 생기는 것으로 추정된다. HPV-16 및/혹은 HPV-18은 미국과 유럽에서 HPV 관련 여성기 전암 환자의 약 80%를 일으킨다. 여성기 전암이 생기면 희귀하지만 저절로 낫기도 하는데(예컨대 동 병변의 자연적인 해소) 그 비율은 1.5%에서 5%까지이다. 미국에서는 매년 약 6천 명의 여성기암 환자가 새로 생기는 것으로 추정되며 그 중 약 50%에서 80%는 HPV가 일으킨다.

연구 개요
여러 병원에서 진행한 본 오픈 레이블, 무작위 2상 연구는 HPV-16 및/혹은 HPV-18이 일으키는 전암성 여성기이형성증을 갖고 있는 18세 이상의 성인 여성에 대한 VGX-3100의 안전성 및 효능을 평가하기 위해 설계되었다. 평가된 핵심 효능 측정치들은 HSIL 해소, HPV 16/18의 미검출, 병변 면적의 감소 등이다. 33 명의 여성 환자들이 0, 4, 12 및 24번째 주에 VGX-3100 네 도스를 주입 받았으며 그중 8명은 보조 치료로 평가되고 있는 국부 이미퀴모드 치료 또한 받았다. 이미퀴모드 치료는 VGX-3100만의 효능에 대한 유의미한 개선이 없었기 때문에 중단되었다. VGX-3100 치료 이후 6개월 시점에서의 효능은 데이터가 확보된 모든 참가자에 대해 보고될 예정이며 이들은 VGX-3100만을 주입 받은 환자 22명이다. 참가자 세 명은 이동(1명)과 조기 수술 대상자로 선정(2명)되어 중도 탈락했다. 본 시험은 4번째 도스 투여 이후 1년까지 치료를 받은 환자들을 대상으로 진행된다.

이노비오의 HPV 관련 DNA 의약품 임상 프로그램

VGX-3100

INOVIO Interim Results of an Open-Label Phase 2 Trial of VGX-3100 Show Efficacy Against HPV-associated Vulvar Dysplasia

PLYMOUTH MEETING, Pennsylvania, March 30, 2020 /PRNewswire/ -- INOVIO Pharmaceuticals, Inc. (NASDAQ: INO) today reported interim results from an open-label Phase 2 trial designed to evaluate the safety and efficacy of VGX-3100 in women with vulvar dysplasia, also known as high grade squamous intraepithelial lesion (HSIL), a precancerous condition caused by high-risk human papillomavirus (HPV) types 16 and/or 18. A total of 22 subjects have received all 4 doses and will be evaluated to the end of the study. INOVIO is reporting interim data on the 10 subjects who have completed their primary endpoint evaluation at six months following treatment with VGX-3100. Based upon the available results, 80% (8 out of 10 subjects) had a reduction (2 cm2 on average) in qualifying lesion area (average 60% reduction) of which 20% (2 of 10) completely resolved their vulvar HSIL and had no virus detectable in the healed area, 6 months following treatment.

Trial results will be presented from April 1st through April 2nd at the annual American Society for Colposcopy and Cervical Pathology (ASCCP) meeting as a virtual session titled: "Preliminary Results of an Open-Label Phase 2 Study of VGX-3100 for the Treatment of HPV-16 and/or HPV-18 (HPV-16/18) Related Vulvar HSIL." Safety results were consistent with the known safety profile of VGX-3100. There were no drug-related serious adverse events. No cases of carcinoma have been observed.

Vulvar HSIL is an orphan condition that usually requires surgery, which has significant physical and psychosocial impacts (e.g. severe pain, disfigurement, sexual dysfunction). The success of such surgery is marginal, as the recurrence rate of high grade vulvar pre-cancer is approximately 30 to 50% three years post-treatment. Overall less than 5% of women with vulvar HSIL exhibit spontaneous resolution. Without adequate treatment vulvar HSIL can progress to vulvar cancer which is projected to claim approximately 1350 lives in the US in 2020 alone.

VGX-3100 Safety and Efficacy Highlights for Vulvar Dysplasia

About Vulvar Dysplasia (HSIL)
If left untreated vulvar pre-cancers can progress to invasive cancer of the vulva. Approximately 27,000 cases of HPV-related vulvar pre-cancers occur in the U.S. each year with a similar number of estimated cases in Europe each year. HPV-16 and/or HPV-18 are involved in about 80% of HPV-related vulvar pre-cancers cases in the U.S. and Europe. Once vulvar pre-cancers develop, spontaneous regression (i.e. natural resolution of the lesion) is rare and occurs in 1.5% to 5% of cases. An estimated 6,000 new cases of vulvar cancer occur in the U.S. each year with about 50% to 80% of those being HPV-associated.

About the Study
This open-label, randomized, multi-center Phase 2 study is designed to evaluate the safety and efficacy of VGX-3100 in adult women 18 and older, with precancerous vulvar dysplasia caused by HPV-16 and/or HPV-18. The key efficacy measures being evaluated are resolution of HSIL, non-detectability of HPV 16/18, and the decrease in lesion area. 33 women received 4 doses of VGX-3100 at Weeks 0, 4, 12, and 24, of whom 8 subjects also received topical imiquimod, which was being evaluated as an adjunctive treatment. Enrollment into the imiquimod arm was halted due to having no meaningful improvement upon the efficacy of VGX-3100 alone. Efficacy at 6 months following VGX-3100 treatment will be reported for all participants with available data, 22 of whom received VGX-3100 alone. Three participants have discontinued due to reasons of moving (1 subject) and electing for earlier surgery (2 subjects). The trial will follow treated participants for up to 1 year after the 4th dose.

About INOVIO's HPV-Associated DNA Medicines Clinical Programs
This study builds on significant clinical benefits demonstrated with INOVIO's HPV-associated DNA medicines in multiple clinical trials. Specifically, VGX-3100 in a Phase 2 proof-of-concept trial for cervical dysplasia demonstrated a complete response in 43 out of 107 patients in regression of high-grade cervical lesions and elimination of the underlying HPV infection. Additionally, two out of four metastatic HPV-associated head and neck cancer patients treated with MEDI0457 and a PD-1 check point inhibitors in a Phase 1 study experienced a long-term complete response for more two years and counting. Lastly, a pilot study of INOVIO's DNA medicine INO-3107 in recurrent respiratory papillomatosis (RRP) resulted in two out of two patients delaying surgery due to lack of tumor recurrence.

About VGX-3100
VGX-3100 is a DNA medicine under Phase 3 investigation for the treatment of HPV-16 and HPV-18 infection and precancerous lesions of the cervix. INOVIO is in open-label Phase 2 clinical trials evaluating its efficacy for treating HPV-related vulvar and anal dysplasia (HSIL). VGX-3100 has the potential to be the first approved treatment for HPV infection of the cervix and the first non-surgical treatment for precancerous cervical lesions. It works by stimulating a specific immune response to HPV-16 and HPV-18, which targets the infection and causes destruction of precancerous cells. In a randomized, double-blind, placebo-controlled phase 2b study in 167 adult women with histologically documented HPV-16/18 cervical dysplasia (CIN2/3 or HSIL), treatment with VGX-3100 resulted in a statistically significantly greater regression of cervical dysplasia and clearance of HPV-16/18 infection vs. placebo. The most common side effect was injection site pain, and no serious adverse events were reported.
